Register in person
Please pop into the surgery and pick up a patient information sheet. Once you have filled in the form please return it to the surgery, please fill in as much information as you can. We will write to you within 7 working days with a registration GMS1 form which you must bring in person to the surgery. If however, you need to be seen urgently please speak to the receptionist. Alternatively, if you prefer you can download the forms and bring them with you to the Surgery.
All new patients over 16 years of age who register with the practice attend a 10-minute medical examination. Your previous records will not arrive for some time, so this is also an opportunity to discuss your medical history and medication. Any patients on medication will need to book an appointment to see a doctor for thier first issue of medication.
As a small practice you will be registered with North Avenue Surgery and you can be seen by any of the doctors that are working at the surgery, however you’re named Accountable GP will be Dr N K Shah.
Please bring along your Medical Card. If you do not have a Medical Card, you will be asked to produce a form of identification to confirm the correct spelling of your name, address and date of birth to prevent errors and to ensure you are living in our catchment area. (Please refer to our registration policy below).
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.
Your medical records may take several weeks to arrive at the Practice.

The National Care Record Service (NCRS)
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
